Is this your first time on pointe or are you having a difficult time getting a good shoe?  I would also bet you have some hyperextension in your knees which may be adding to your problem of getting up on the box.  If you ever watch dancers, those with hyper extension in the knees definitely have pronation of the ankle over the toes on pointe.

It is difficult to say what to try.  I can say if the shoe doent feel good or tight in any way when you first try it on it is probably the incorrect shoe.  If you can get it to feel good with a very little breaking in it may be a decent shoe.

Do you live close to a place with a good dance store (like New York City)?  It may be your best bet to go there (or another city) and try on shoes at the different stores as they have (most) all sizes and styles in stock.  This way you can compare/contrast without having to pre-order every shoe in.  I would think most major cities with a decent dance store (possibly specializing in ballet) would offer you this opportunity and have a good "fitter" to help you along.

Long toes mean high vamp (short toes low vamp), broad foot means broad box (narrow foot means narrow), strong foot means weaker or shorter shank (weak foot means stronger and longer shank), then they offer shoes for different shape feet, egyptian, grecian, and somewhat of a "normal" foot.

I still suggest you go see a dance therapist, most colleges and universities and most (all) professional dance companies may have one you can go see.  They will help you with your hyperextension problems and even work you through your releve/eleve on pointe and help you articulate and strengthen the proper muscles in a way even a great dance teacher would probably never investigate or articulate.
